Admission process
- Adult Medical Day Care is for individuals who need health
and wellness services while living in the community - A nurse will provide an initial assessment at no cost
to you to determine the appropriateness for attending the program
Rates
- We offer an all inclusive rate for 5 hours of health and wellness
- For those clients who need more than five hours, additional hours may be available
- This rate includes
- all the medical services (nursing, social work, dietary consultations, therapeutic recreation, physical therapy occupational therapy and speech language pathology, visiting physicians and specialists)
- hot nutritious meals and snacks
- entertainment and periodic outings
- hair salon, beauty salon, personal care
- transportation to and from the facility
- transportation to and from doctor appointments is available
Additional Funding sources
Funding is available for qualified clients under many insurances and entitlement
programs
- NJ Medicaid and all Medicaid HMO programs will cover
this service for qualified clients. Eligibility depends on the clients
need for this service. A registered nurse will determine the client’s
eligibility via a health assessment interview. Please call our center
to arrange for a confidential nursing assessment. - State and County Waiver Programs such as:

- Traumatic Brain Injury (TBI)
- Jersey Assistance for Community Caregiving (JACC)
- Veteran Affairs (VA)
- Aide and Attendance Program for Veterans and Spouses of Veterans
